AliveCor is making available to the patient community 500 FREE AliveCor Heart Monitors for iPhone4/4s or iPhone 5.

To receive your monitor, you  will need a prescription for the device from your doctor. All that AliveCor asks in return is for patients to take an ECG (electrocardiogram) reading once a day for several months. Individual data will be kept confidential; collectively, it will add to the quarter-million ECG readings in AliveCor’s growing database, enabling AliveCor to deliver insights back to patients.

The AliveCor Heart Monitor is an easy-to-use, powerful, and accurate ECG-recording device that fits in your pocket and enables you to record an ECG anywhere, anytime. If you feel a symptom, the Heart Monitor can capture it quickly. The monitor is FDA-cleared in the US for use by medical professionals and patients (by prescription only) to record, display, store, and transfer single-channel ECG rhythms.

  1. If you haven’t already, obtain a prescription for the “AliveCor Heart Monitor for iPhone 4/4S” or “AliveCor Heart Monitor for iPhone 5″ from your physician*. You can download and print the product sheet here.
  2. Submit your prescription here, and include the following:
    1. Your doctor’s name, address, phone number, license number, and license state or NPI (National Provider Identifier)
    2. A copy of your prescription (a scanned copy or photo is sufficient) ¾ click on “Attach File” at the bottom of the page to attach the copy
  3. Once your prescription is submitted, click here to order your FREE monitor.
