The Caregiver Action Network (CAN) recently launched a new program, Advancing Excellence: Best Practices in Patient and Family Engagement, to identify and recognize best practices in patient and family engagement.
There is excellent work being done in patient and family engagement, and the Caregiver Action Network aims to recognize leaders in patient-centered care and share these stories widely to help improve the quality of care across the country.
Please consider submitting a nomination for a patient or caregiver, hospital staff member, or healthcare leader working in patient and family engagement. They can be involved in atrial fibrillation or any other area.
The deadline to apply is May 23, 2014, and nomination forms are available at www.CaregiverAction.org (look for the blue Nominations Form button, then click on links for hospital/staff or patient/caregiver nomination forms in the upper right).
